SCHEDULING AND
PAYMENT
If you are interested in scheduling an Integrative
Medicine consultation please complete the following steps:
Appointment set-up:
1. Please read the description on the
appointment page for Integrative Medicine consultations or on
the mind/body medicine
page for Mind/Body consultations.
2. After reading the consultation description please call 231-439-0905
to set up an appointment for your initial consultation visit.
3. After you have scheduled your appointment please download the
patient intake
. Mail your completed intake form to:

4. Please mail the forms at least seven to ten days prior to your visit
so we are sure to receive them. Although the forms may seem lengthy, the
more information you can provide for us, the better we will be able to
create a complete treatment plan for you.
5. Obtain medical records from your primary physician and bring copies
to your initial visit.
6. Please hand-carry all vitamins, supplements, and prescription drugs.
7. Please compose a Patient Summary that expresses how you are doing on
a mind, body, and spirit level. You may hand carry your summary or mail
it along with the patient form.
Payment:
Payment will be expected on the
day of your visit. Checks and credit cards will be accepted. The total
cost of the full Integrative Medicine assessment and consultation is $625.00.
This will include two visits and a written treatment plan. The following
schedule and fees will be applicable:
a. Initial visit- The initial visit
will last 90 minutes. At this time an extensive review of your history
will be taken. Payment due at this time will be $400.00.
b. Follow-up to initial visit- The follow-up visit will last 60 minutes.
At this time, you will receive a written treatment plan which will be
discussed with you at length. Payment due at this time will be $225.00.
c. Further follow-up visits will last 30-60 minutes and will be charged
at $165.00/hour.
d. Any additional medical literature research you may request can be done
and will be charged an hourly rate of $165.00/hour. This will include
literature searches of several medical databases including Medline.
The cost for an initial mind/body
consultation is $275.00. Follow visits will be scheduled for 30 to 45
minutes and cost from $85.00 to $125.00.
We do not accept payment from or bill insurance companies. However, you
are welcome to submit the receipt you receive from us to your insurer
yourself.
Primary care policy:
Pursuit Health offers a consultation service and does not provide primary
care services including diagnostic testing. For this reason it is strongly
recommended that you have your primary care established with an internist
or family practitioner. In addition, Dr. Aikens is not available to take
emergency phone calls. All calls will be answered within 24 hours of being
received.
